Avianca LifeMiles is great for Star Alliance awards with no fuel surcharges. Amex is also the only major program that transfers to Delta SkyMiles.

US Amex charges an excise-tax offset fee of $0.0006 per point (max $99) only on transfers to US programs — in practice, Delta and JetBlue. Transfers to foreign programs have no fee.
